Warranty of Treatment

A warranty in health care is almost unheard of! At the Center for Dental Health, we have built a solid practice based on customer loyalty, trust, and endless satisfaction. Confidence and experience allow us to offer this extra peace of mind.

Our team is proud of the dentistry we provide and we are confident in the durability of our treatment. Our ultimate goal is to provide you with a durable health and pleasing smile. However, there are many factors which play into this accomplishment – some of which we are not in control Part of our approach is to show you how to prevent future dental problems. The long-term success of your dental condition depends upon how well you follow treatment recommendations and how well you take care of your teeth.

Regular exams, cleanings, fluoride treatments and professional supervision and oversight are an essential part of dental healthcare. Your individual condition depends on the frequency of continued care visits and the products we recommend for you. With that in mind, we are very pleased to offer you the following dental treatment warranty at The Center For Dental Health.

CROWNS, BRIDGES, INLAYS, ONLAYS, AND PORCELAIN VENEERS

If these dental restorations fracture with normal use during a period of five years from the date of their initial placement, we will replace or repair them at no additional charge. (Note exclusions and disallowances below).

COMPOSITE BONDING OR FILLINGS

For composite (tooth colored) bonding or direct-placed fillings, we will replace or repair them in the event of failure for a period of two years from the date of their initial placement.

DENTAL SEALANTS

Sealants are plastic coatings placed on the chewing surfaces of the teeth to prevent tooth decay in the pits and grooves. These are the most common areas to get cavities. Flossing and the use of fluoride after their initial placement will help prevent tooth decay between teeth. We will repair or replace any of our sealants that fracture or break for a period of 18 months from the date of their initial placement.

CONDITIONS of DENTAL TREATMENT WARRANTY

To keep this warranty valid, you must:

1) maintain your financial account current and in good standing; and

2) keep your prescribed regular continuing care appointments (with no appointment varying more than 30 days of our recommended schedule); and

3) have all recommended dental treatment(s) performed or underway as recommended by Dr. Lee Ostler (with the exception of treatment specifically recommended elsewhere).

This warranty does not cover procedures not mentioned above, including root canal therapy, periodontal therapies, sleep apnea appliances, TMJ related appliances, and functional appliances such as night guards; nor does it cover damage to teeth or dental prosthesis or appliances caused by trauma, accidents, abuse, neglect, or improper use (e.g., clenching, grinding, chewing ice, or biting on non-food items).
